POLICIES AND ACCESSIBILITY

The Orangeville Foot Clinic is located at 15 Elizabeth St, U-12 (2nd floor), Orangeville. There is plenty of FREE parking onsite, an elevator in the building, and wheelchair-accessible washrooms in the hallway (ask us for the keys).

Referral

You DO NOT require a doctor referral to book an appointment. Also, most insurance companies do not require a prescription from your family doctor for our services and products such as orthotics. Our chiropodist’s signature is enough. However, if you have a referral from your family doctor or other health care providers, please bring it with you to record it in your file.

FEES

Our fees are within the accepted fee schedule as set by the Ontario Society of Chiropodists and the Canadian Federation of Podiatric Medicine. Appointment fees may vary depending on the length of the appointment, and the type of services and products that are provided to you. If you have any questions regarding your appointment fee, please do not hesitate to ask us.

For your convenience, we accept Visa, MasterCard, debit, cash or cheque.

Orthotics

Custom Orthotics may be recommended for your care. This treatment is non-refundable. Orthotic costs include the custom-made device, scanning of the feet, fitting and dispensing appointments, and up to 3 follow-up appointments that may involve adjustment and modification to ensure the best outcomes.
The initial appointment which may include biomechanical assessments and gait analysis will be charged separately.
This is possible that new orthotics need some adjustment after the patient starts using them. There is no charge for the adjustment 3 months after the orthotics are dispensed. For any adjustment after 3 months, a fee may apply.
Insurance companies do not accept direct billing for custom-made orthotics. Therefore, patients are required to pay upfront and then submit their claims to their insurance company.

The orthotics will be cast, prescribed, checked and dispensed by a licensed chiropodist.

EXTENDED COVERAGE

Many extended health insurance plans cover chiropody services, as well as DVA and NIHB. However, it is your responsibility to consult your insurance carrier regarding your coverage before the appointment. If you are unsure of what type of information is required, please contact our office (519-942-4705) and we will be happy to help you.

Chiropodists are not covered by OHIP.

Greenshield Insurance Cardholders and other Insurance policies may be eligible for 3rd party billing (direct submission).

Fees for services rendered are due on the treatment date.

ADDITIONAL INFORMATION

The office is available to answer your calls during regular business hours. After business hours please feel free to leave a detailed message and we will return your call the next business day.

Consent Form

Chiropodists are required to advise patients of the general risks associated with common services and obtain consent for assessment and treatment before initiation of services.
Chiropodists are legally mandated to obtain verbal and written consent for foot examination and treatment.
Consent forms will be updated annually to ensure compliance with the College of Chiropodists of Ontario documentation requirements.
The chiropodist will assess your current foot condition and evaluate your individual risk factors; taking medical conditions, overall health, and activity level into account.
A management plan will be presented to you based on that assessment.
Verbal consent will be obtained on an ongoing basis for routine care and services. Treatments will only be initiated after expressed verbal consent.
Consent will include disclosure of costs associated with treatments, products, and devices before their delivery. Some treatments will involve the use of sharp instruments (nail cutting, callus and corn debridement, wart debridement etc.). During these procedures, there is a small risk of discomfort, including a rare but sometimes unavoidable bleeding point. Treatment will include management to prevent complications and infection control protocols are in place to protect patients.
In all cases, if you are unable to accept the suggested management plan alternatives will be presented.

 

Photos/Videos

Occasionally, photographs of your feet will be taken with your ongoing verbal consent for the following purposes:

  • Documenting initial foot condition,
  • Documenting treatment (before/after),
  • Monitoring your foot condition and evaluating treatment success,
  • Educational purposes on social media and the clinic’s website (Non-identifying images ONLY).
Verbal/written consent will be obtained for the above purpose.

Cancellations/No-Shows

Cancellation/No-show Policy: 50% of the appointment fee will apply for any cancellation with less than 24 hours’ notice and full appointment fee for no-shows.
